Employer Failed to Withhold Federal Taxes

I file head of household. I'm single with one dependant. I've worked for my employer 2 years. I submitted my w4 in 2023 when I started. Federal taxes were withheld in 2023 as they should have been. I NEVER changed my w4. This is their screw up. I was shocked receiving my W2 and discovering federal taxes weren't withheld all of 2024. Local Oregon state taxes were withheld. 

Box 1, 3 & 5: $19741.16

Box 4: $1223.95

Box 6: $286.25

Box 17: $291.71

Beyond filing as is, what can I do? Report my employer?

Employer Failed to Withhold Federal Taxes

As an HOH, your standard deduction would be about $22K. With taxable wages (Box 1) of about $20K, you would have no taxable income for federal purposes, so zero federal withholding is correct. if you want a refund, thus making an interest-free loan to the IRS during 2025, or have other taxable income, submit a new w-4.
