"Duplicate Entries for Boxes 18-20"

Hello!

I have two errors of "Duplicate Entries for Boxes 18-20" and I'm not sure what I'm supposed to change. I have the same issue for both errors. On my W-2s, it has two of the exact same number in box 18, stacked on top of each other on separate lines. In box 19, there are two different numbers, separated on different lines the same way. The locality name only has one name. I have tried to show what it looks like below (please ask questions if it is confusing). I am reviewing my taxes and it says there are duplicate numbers. I am confused. In searching, I've found that Google says Ohio's rules are different for these boxes (all my taxes are from Ohio), but won't let me know what I'm doing wrong. What do I do?

contact the issuer. those boxes do not make sense

18 is the locality wages

19 is the same locality's withholding. 

it's not realistic for withholding to exceed wages or for withholding to be almost as much as wages. Also locality income tax rate is generally less than 3% of wages.
