Do I need to mark each W-2 for PFL if I was paid out by a third party insurer?

I got Covid in beginning of 2022 that caused me to be out on FMLA. I purchased optional short term and long term disability from my employer. Thankfully I was able to get paid through that 3rd party insurer. But, I have 3 W-2's now. I read that I should mark that I got paid family leave, but not sure what that entails. I have one W-2 from employer with regular pay. One W-2 from employer coded as third party pay for my short term disability. Now another W-2 from the insurance company for my long term disability.

Do I need to mark each W-2 for PFL if I was paid out by a third party insurer?

Yes, you need to indicate that your income is PFL, if that is how your W-2 forms read. Enter your W-2 forms exactly the way they are printed. 

 

In your situation, you may have entries for PFL in box 12 or 14. Enter this information exactly the way it is on your W-2 form.
