I am retired on Disability and receive a 1099 from OPM (OFFICE OF PERSONEL MANAGEMENT

) BLOCK  2a has unknown entered. I went to the page that your system recommended for the answer

now I am more confused than before. i believe i should be using the Simple method.  but the firt thing they want is the final payment date from form 1099.  where on the form can you find the date they want and how long is the annuity for? i do not know that either. please help.

You need to contact the plan administrator for any needed information. These will not be on the form itself. You may have gotten letters from OPM explaining this.
