Are long term capital gains part of the progressive tax bracket

Are bank interests, stock dividends and long term capital gains included in w2 box 1?

 

Should bank interests, stock dividends and long term capital gains be included in the gross pay on w2 when calculating the social security and medicare taxes (and additional 0.09%)?

 

Should long term capital gains tax (taxed at 15%) be added to the final tax liability or should it be part of the total taxable income and use to calculate the withholding with the tax brackets? (The difference between the two calculations is very significant.)
