8815E-T form

The T stands for taxpayer and the 8915E is for disasters from last year. If you did not take a disaster distribution and do not need the form, follow the steps here. If you did take a disaster distribution and are paying taxes on the next 1/3 of the money, it needs to be reported on the 8915-F, which is for this year. The E was for 2020 filing of disaster distributions.
