My husband received a lump sum that has a taxable amount in box 2a however box 4 says there is no federal income tax withheld. It does show 4D in the distribution code but IRA/SEP/SIMPLE is not checked. There is an amount in State tax withheld. I've never had to input this before so I'm not sure where exactly I need to fill in this information.

Under 'Wages and Income' scroll down to 'Retirement plans and social security'.
Click on the tab marked 1099-R and enter the information just like you do for your W2s.
Ok, even though its not a retirement plan for my husband per say? It just counts as income for him?
It is a retirement plan for him - an inherited retirement plan. So, yes, enter it in the same place.
