1099-R death benefit taxable for Federal and State?

I received a 1099-R for a one time lump sum death benefit for my father.  Box 1 and 2 are identical to the amount received.  No federal income was withheld.  Box 7 has code 4.  But there are no entries for boxes 14-19.  I'm assuming it is taxable for the IRS but is it taxable for the state (CA)?
