Who is the "payer" for the 1099-SA. My form is from Bank of America, so is it their info I enter in teh various "payer" boxes? And is the Federal ID number the same as the "Payer's E.I.N"?

Yes, Bank of America would be the Payer.  Also, the Payer's EIN would be the Federal ID and it should be in the following format XX-XXXXXXX.
