Where does a T5008 Form from a Canadian ESOP belong?

I have an ESOP from Computershare Trust Company of Canada and can't seem to find where it belongs.

Where does a T5008 Form from a Canadian ESOP belong?

You use the "Stocks, Mutual Funds, Bonds, Other" interview and tell TurboTax you did not get a 1099-B.

 

The resulting interview elicits the 5 pieces of information that comprise the security sale reporting on Form 8949:

  1. Description of what was sold
  2. Date of sale
  3. Proceeds
  4. Date of acquisition
  5. Cost basis.

To enter your investment:

  1. Open your TurboTax account > Wages & Income.
  2. Scroll to Investments and Savings (1099-B, 1099-INT, 1099-DIV, 1099-K, Crypto) and Edit/Add or Update.
  3.  On the Your investments and savings screen, scroll down to Add investments. 
  4. Continue and on the Let's import your tax info screen, select Enter a different way. 
  5. On the OK, let's start with one investment type screen, select Other
  6. Enter the details and continue with the onscreen  interview until complete.
