This is for Kat (level 15). I know I enter the medicare premiums in medical expenses under deductions and credits. Which line item in Medical do I use?

Do I add my medicare premiums to my other medical insurance expenses and lump them together in one entry?   I cannot place it in the spouse column on the worksheet, as that entry tied to a Form SSA 1099-SM of which I do not have one.

Yes, you would enter these in the medical expense section under medical insurance premiums. Here you will add your premiums and your wife's premiums together to report.
