Reporting Excess HSA Withdrawal

I made a Post-tax direct contribution to my HSA which was an excess, due to Medicare enrollment. I withdrew the excess and the corresponding gain within the IRS time limits. How do I report it on Form 8889? Should I use any other form(s)?

excess contributions withdrawn by the due date get reported on line 14a of 8889 for the year made.  any earnings are taxable in the year of withdrawal.

The excess contribution amount is not to be included in Line 2, correct? It was a direct payment from post-tax funds.
