Qualifying Charitable Organizations

I made multiple donations during the year to three different Arizona Qualifying Charatible Organizations. Do I need to list the individual contributions and dates or can I provide a total and the date of the last donation?

I work for a 503(c)(3) organization which is eligible to receive tax-deductible charitable contributions. Does this mean I get all my federal income tax withholding back?
