For miles driven to get medical care, do I enter just the miles driven to the destination or do I count the miles for the round trip to get back home?

You may claim the miles going to the destination and back home.  

It is a good idea to keep a log to record your medical trips which includes the date, the destination, and the purpose of the trip.  You can also claim any parking fees or tolls that you had to pay when driving for medical care.
