If 100% of the HSA was not spent, but the remaining was rolled over into the same account, is this a rollover in the HSA question?

No. You are not required to spend the entire account balance. There is no penalty for letting the unused amount accumulate.
A HSA rollover is moving the balance of one HSA to another HSA.
