I moved and my 1099-sa form has my old address do I change it on your form?

When you are entering the information from form 1099-SA, and all other forms, use the address on the form, even if it is incorrect.   It needs to match what was sent to the IRS.  The address you entered when you began, under  the 'personal information' section is where you want to enter your current address.
