That form is part of your state interview however you must print in sign and mail the return in. It is not part of the e-filing process.

If you have already e-filed the federal or state returns then to complete this form you'll need to log in, scroll down and click on add a state to open the archive return. then complete the state interview again all the way to the end and follow the instructions for filling out that form. Then to print the form click on print center under the tools option on the menu on the lower left.

There is no additional charge for the form, if you did your NC return with TurboTax (TT).

You cannot do that form, in TT, without doing both your federal and state return in TT.
