How many months does a child have to live with you and does it have to be the last 6 months out of the year or any 6 months?

Does it have to be the last 6 months of the year to claim a dependant or can it be the first 6 months of the year??

More than half the year means the child lived with you more than half the nights of the year, for 2015 there are 365 days so that means 183 or more nights that the child lived with you.  They do not have to be in one block of time.  You can share custody back and forth every week, and then count up the number of nights the child lived in each home.

She lived with me until December 3rd and moved out. So that means I can claim her?

If she is under 19 (meaning 18 or less) or if she is under 24 (meaning 23 or less) while also a full time student.  If she is older, different rules may apply.
