Form 3520, how to attach statement on Part IV, question 54

I'm filling out tax form 3520, Part IV, question 54,  and I have more entries than there are lines on the form. The instructions say "If more space is needed, attach a statement."

How do I attach a statement? Is there some standard format to use when doing so?
