here are the instruction for that form ( form 8233 ) -->   Instructions for Form 8233 (Rev. October 2021)

However from your post I am not clear as to what you are trying to do.   Are you an withholding agent ?  or are you an employer  ?   Assuming that you are a US person ( citizen/ GreenCard / Resident for Tax Purposes ), are you employing another US person or what ?

Please tell  more about the situation and what you are trying to do, including  the tax treaty involved .
