Yes. The cost for modifications to the van, to make it wheelchair accessible, is a medical expense which would be deductible on Schedule A of your return, if you itemize your deductions. You can include in medical expenses the difference between the cost of a regular car and a car specially designed to hold a wheelchair.

Is there an itemized schedule I need to detail all of the various components ?
Yes, you can provide descriptions along with amounts.
With your return open, search for Schedule A and then select the Jump to schedule a link in the search results.
Proceed through the interview until you come to the Tell us about any other medical expenses screen. There, you'll be able to enter descriptions and relevant amounts.
