I have worked in Japan for the last 7 years. I cashed my employers 401K and received a 1042S from Fidelity Investments Institutional Operations Co.

Where do I enter the amount in Turbo Tax? Box 1. Income code: 15,  Box 3: 3, Box 3a: 00, Box 3b: 30.00, Box 4a: 15, Box 4b: 0.00.
