What would I put for the places I lived section? if I am from PA but in Aug of 2019 I moved to TN for work but kept my PA ID, then in Jan. of 2020 I moved back to PA.

You should file a part-year resident return for the state of Pennsylvania since you did not live in the state for all of 2019. You do not need to file a Tennessee return since that state does not have an income tax.
