New York State Taxes Not Calculating Properly

When I am inputting my state taxes into Turbo Tax, it is not calculating my state tax rate properly. It is calculating my entire amount owed off of my highest tax bracket rather than a blended rate off of each marginal bracket. Has anyone else experienced this issue?

What is your filing status on your New York tax return?

What are the exact amounts on Form IT-201 lines 33, 38, and 39, or Form IT-203 lines 32, 37, and 38? Look at the actual tax form, not a summary screen in TurboTax.
