Hello!
Upon submitting my return, I was given a pdf copy of the MCTMT Worksheet for Self-employed individuals. I was also given the notice, "Important: Your taxes are not finished until all required steps are completed." But it didn't give me an address to mail anything to.
In my research, I have found many posts about this going back several years, and yet somehow I can't find a straight answer. All I have found is the following address, but no guidance:

SO, my questions are these:
1. do I print and mail just the worksheet along with payment for the MCTMT balance? or include a copy of my entire IT-201?
2. will by MCTMT balance due be negated by the fact I have a sizable state return on the way?
3. do I need to even do anything?
For additional context, I filed state and federal in mid-February. I have since received my federal refund, but I have yet to receive my state return 6+ weeks later... which leads me to believe I do need to do something, as I've never experienced it take so long to receive a NYS refund...
