I moved to New York City end of August and need to submit the 1127.2 form for a refund for part-time resident, I only have the 1127 form. Where do I get the 1127.2?

Form 1127.2 is attached to your W-2, or it's an additional copy of your W-2. It shows your Section 1127 wages and the amount of 1127 tax that was withheld. (That information might be in box 14 of your W-2.) You get Form 1127.2 from the employer. If you file your Form NYC-1127 by mail you have to attach Form 1127.2 to the NYC-1127.
