This year, Gerber Life sent me a 1099 R. The 1099 R has a taxable amount and I tried entering this information into TURBO TAX. TURBO TAX wants me to identify the State Type code for this form.
TURBO TAX thinks this is PA retirement income.
So what code do I select from the dropdown list of available codes?

Assuming that this was either a distribution of cash value or a surrender of a life insurance policy, I would select "Life insurance or endowment" given these choices for a payout from Gerber Life.
