Do I include non-taxable disability payments when asked for "other non-taxable PA income" on my state return?

It's the section to see if I qualify for special tax forgiveness.  Disability payments were made to me by a third party sick pay insurance plan.

No. Sick pay and disability, including payments by 3rd party insurers, is not added in determining your PA Special Forgiveness Tax Credit.
