If my wife was paid with cash once a week for four months to watch her niece, do we report that as self-employed income?

The total amount was $2,040 and was once a week from January to May.

If my wife was paid with cash once a week for four months to watch her niece, do we report that as self-employed income?

Yes, that income would be reported on Schedule C Business Income and Expenses as self-employed income.  Report the total income as 'cash' and include any applicable expenses related to the income.
