When I changed jobs my 401k was directly rolled over to a traditional ira.

When I changed jobs my 401k was directly rolled over to a traditional IRA. After I enter all of my 1099R info, turbotax asks "Where is this distribution from?". The only choices that I would think apply are Pension/IRA distributions received before retirement age OR None of the above. Which one would I choose? Also, there is a box for Michigan Distribution Amount and also Michigan Withholding. What, if any, values do I input here?

choose pensions/IRA. your 1099 should show Code G in box 7. did you enter it correctly?

Yes, I entered Code G. So, now do I enter the Gross distribution amount(Box 1) from my 1099R for Michigan Distribution Amount and then $0.00 for Michigan Withholding since the Taxable amount is $0.00(Box 2a)?

please enter the 1099 exactly as it is printed. don't embellish.  then answer the followup questions.
