WI medical insr=urance premiums deduction

I see this going through questionnaire on Premier but

doesnt have a place to input 

medical premiums paid from Social Security

or 

I pay ~$!500 a month for insurance

You would enter medical insurance premiums on your federal tax return in the Deductions and Credits section, then Medical, then Medical expenses. It will flow down to your state tax return. The Medicare premiums would also be entered there unless you paid them from your social security benefits, then you would enter them when you enter your social security benefits reported on your SSA-1099 Form:
