Where can I find the plan cost at annuity starting date for a pension? Form 1099-R does not show this information.

Appears this information is needed for the Simplified Method. First year with a pension payment.

The amount of the employee contributions should be found in box 9b of the CSA-1099R Civil Service annuity form.

If the employee contributions are not found there, you will have to contact the Office of Personnel Management.
