it depends. Did you have a prior year Social Security Lump Sum payment or do you have one this year?

No, with a tax prep company.

Check your return you received from them last year and look for a SSA-1099 worksheet or Social Security worksheet. It should have the information there. If you don't have it, go to the tax preparation company and ask them for a copy of your return that includes the Social Security Worksheet.
