What do I do if I don't have a 1099-R for an IRA withdrawal and cannot get one?

The account was registered as an outside trustee plan and Putnam was not responsible for any tax reporting.  As such, no taxes were withheld but a 1099-R was not issued so I can't properly complete the tax return.

What do I do if I don't have a 1099-R for an IRA withdrawal and cannot get one?

You can enter a substitute Form 1099-R in TurboTax to report your pension distribution that is not reported on such a form.

 

To enter a substitute form 1099-R in Turbo Tax follow these instructions:

 

  1. Find the Retirement Plans and Social Security menu option
  2. Start or Update IRA, 401(k), Pension Plan Withdrawals (1099-R)
  3. Choose Add Another 1099-R
  4. Choose I'll type it in myself
  5. On the screen that says Tell Us Which 1099-R You Have choose I need to prepare a Substitute 1099-R

If your distribution is a normal distribution, not subject to a penalty for early withdrawal, you can enter code "7" in box 7. The distribution amount goes in box 1 and the taxable portion of it goes in box 2(a).

The IRA trustee is required to issue a Form 1099-R for IRA distributions.  Only a bank or an IRS-approved nonbank trustee can be an IRA trustee and it can't be an IRA without a trustee.
