we file contract labor for our drivers on the 1099, but an additional $200 is added seperately to help them on the health insurance premiums monthly. How do I report that on the 1099, or do I?

Since you do not pay the money to the insurance company and you don't require it be used for insurance it is added to all the other payments you make to them on the 1099.
