Do I need to report on my 1040 form income/loss from UBTI stocks in my IRA and regular brokerage accounts in addition to 990-T form filed by my broker?

UBTI

No.  UBIT is an obligation of your IRA, paid from your IRA, so it simply factors into the investment performance of your IRA.  It is not your personal obligation, so it goes nowhere on your individual tax return.
