United Welfare Fund provides insurance reimbursements and pension plans, I'm not sure what else.
If it is medical reimbursements, it would just reduce your medical expenses that you can claim.
If it is some type of pension plan you should have received a 1099-R for any distributions made to you. So many companies are electronic these days so perhaps you need to sign on to get a copy of your tax forms.
