You should certainly report any 1099-Div forms which use your name and Social Security number.

You should report the 1099-Div for 'retirement' unless it is in the name of a retirement fund with a Taxpayer Identification Number and not your Social Security number.

You should report the 1099-Div for the 'custodial' accounts unless they are in the name of a unique fund with a Taxpayer Identification Number and not your Social Security number.
