Schwab withheld 10% for federal taxes.  It is not a penalty and nothing about it goes on Form 5329.  The amount in box 4 of the Form 1099-R is credited on line 25b of your Form 1040 to be applied against your overall tax liability to reduce your balance due or increase your refund.
