Turbotax is asking for information from my 1099-R, but the boxes turbotax is asking for do not appear on my 1099-R

it wants boxes 14-16, but these do not exist on my 1099-R

Boxes 14 to 16 of form 1099-R report state tax withholdings. If you do not have have these boxes on your form 1099-R, then leave these boxes blank on the TurboTax entry screen.

Expected a net refund of $8162 (original refund amount less IRS balance and TurboTax fees). Refund received is $6714. Need to know why the difference. Thanks
