It is not necessary to include medical insurance information with your return, unless you have received Form 1095-A.
If you are not paying for your medical expenses with after-tax (out of pocket) income, it generally won't be deductible and you won't need to enter it at all.
If you did pay for the medical insurance through the annunity, you will need to have enough medical and other itemized deductions to exceed your standard deduction for 2019, or you don't need to enter it, as it will not change your tax liability or refund.
