received a 2015 RDM late 4-5-2016 but applied to 2015 turbo tax. Then took regular RDM 11-15-2016. 1099-R shows both amounts because both were taken in 2016. How do I show the correct amount.

The sum of the distributions reported on the 2016 Form 1099-R is income reportable on your 2016 tax return. If you reported any of this as income on your 2015 tax return, you must amend your 2015 tax return to remove it. The only thing that potentially should have been included with your 2015 tax return is Form 5329 Part IX requesting a waiver of the excess accumulation penalty on the late-taken RMD.
