My wife has an IRA with Wells Fargo, I have an Annuity with Delaware Life and 401K with Fidelity...dividends are re-invested...do we need to file a form for this?

If you didn't withdraw funds from any of those accounts in 2017 then there is nothing to file, because no taxable event took place.
