My Ohio W-2 has two lines with different information on each in boxes 18-20. How do I file this info?

Boxes 18-20 on my w-2 have two lines per box with varying information in each. the first line is the wages, taxes, and the locality for the city my job is based in (Mentor.) . The second line lists a different amount of local wages than the first, & says 0.00 for Local Income tax . The locality is a School District, it says 4302 Willoughby-Eastlake CSD. How do I file this? Do I enter the CSD information as another locality?  I have a second w-2 since i work remotely in a different city, but the same school district, & I entered that information as another locality already.

Enter the second line as a locality with its corresponding amounts.  The software will designate this information appropriately.
