Your basis is your cost in the retirement plan.  Cost is the sum of all your contributions into the plan (do NOT account employer contributions or earnings) less the sum of all previous distributions from the plan.  Essentially, you get your contributions back before any distribution is taxable.  You will not find this information on the 1099R.  You either must use your own records or consult with the plan administrator for this information.  There is no set early withdrawal penalty by PA
