Illinois 1099-G

Illinois 1099-G for unemployment compensation missing payer name and address. This is the 1099-G downloaded from the IDES. It has no payer name and address which I need to file taxes on TurboTax. Anyone know where to get it?

My domicile is in South Dakota. I worked remotely part-time on an hourly basis (not contract) for a company located in Illinois. Do I file as having worked in Illinois?
