I received form 5498-QA do I need to file it with my taxes? If so where do I input this information?

Form 5498-QA which shows contributions to ABLE account. Keep this form in your records for future reference. You're not required to file it with your tax return. You'll need this information to calculate your taxable income when you decide to take distributions from your IRA.
