I had voluntary taxes withheld from my SSDI Income. I am mailing my tax return. Do I send my 1099-SM form with my tax return?

On the bottom of my 1099-SM, it states Do not return this form toSSA or IRS ?!?

When you printed your return to file, there should be an instruction page.  If the instructions tell you to mail it, then yes include it with your return.  If the instructions do not mention it or say not to then don't send your 1099-SM with your tax return.
