I had an IRA withdrawal in 2023. Tax was deducted at 30%. I didn't get a 1099-R as my IRA account had wrong info. How do I report tax withheld without 1099-R?

My financial institution didn't provide 1099-R as my US citizenship info was not updated. I am US resident for tax purposes although I reside outside the US.

While waiting for @dmertz , I have some additional thoughts.

 

You can check your income transcript at the IRS.  Normally it would show the 1099-R as long as one was issued, even if you did not get a copy, but because of the circumstances, it might not be there.  Also look for a 1042-S, which they might have sent if they thought you were a non-resident alien.

Also, you definitely need to contact the IRA custodian and get them to issue the 1099-R or issue a corrected 1099-R.  Did they send you a 1042-S instead?  They should cancel that and issue a 1099-R instead.

 

If you are expecting a refund overall on your tax return, there is no actual penalty for missing the filing deadline.  So you could wait until the IRS custodian issues a corrected 1099-R, if there is not one on file for you.  

 

If they issued a 1042-S, I think you can file with that instead.  I'm not sure how that works in the program.  If they issued no paperwork at all, and you don't want to wait to file, then I think you can enter the withdrawal from your own records (I assume you know the amount of the withdrawal and the amount of withholding).  There should be a link in the program for "I did not get a 1099-R".  If the IRS has questions, they will contact you.

I don't have anything to add.  The financial institution is required to have issued a 2023 Form 1099-R reporting this IRA distribution, so contact them.
