I did not received form 1099-R, what should I do?

If you did not receive Form 1099-R, but you had a distribution from a retirement plan or IRA during 2019, then you should contact the financial institution that administers your retirement plan for a replacement Form 1099-R. If you have online access to your account, you can probably print a copy of your Form 1099-R from that account.
